The Brooks Rehabilitation CompleteCare Department offers many specialized services to support patients recovering from joint placement, hip fracture, spinal surgery, or congestive heart failure. Our department aims to improve the care coordination during the recovery process. At the core of the CompleteCare program are care navigators, dedicated nurses or health coaches, who follow patients through their entire 60 day episode of care. We also have a team of dedicated IT developers that work full time on an IT tool that was built in house to integrate data from multiple EMRs and track patients over time.
CompleteCare interns have an opportunity to learn about the Medicare Bundled Payments for Care Improvement (BPCI) initiative, which links payments for the multiple services patients receive during an episode of care. Under the initiative, organizations enter into payment arrangements that include financial and performance accountability for episodes of care. This program may lead to higher quality and more coordinated care at a lower cost to Medicare.
