Overview

Our program was initiated in Feb 2015 and has since created an evidence-based fall and injury reduction program across all Brooks settings. Our goal is to ensure that staff and patients/residents/clients are empowered with knowledge and tools to reduce the likelihood of fall occurances as well as hospital readmissions. We monitor the compliance of staff with the policies and procedures developed as well as continue to monitor developments in research to ensure that up to date information is provided to all.

Responsibilities

Upcoming projects may include :

  • Staff education project – developing multimedia educational materials, assisting in rolling out to all Brooks staff
  • Research project – monitoring # of falls post DC from Brooks settings, analyze data and initiate quality improvement activities
  • Meditech update - assist with roll out of fall related screens to IP staff
  • Safety Rounds - assisting with monitoring of high risk patients/residents in IP, skilled and residential settings
  • Stepping On - assist with administrative tasks for community fall prevention program

 

To be discussed, possibly Mon-Thurs 9-4. Must be able to travel between settings on a daily basis.
