Supervise delivery of resident care provided by licensed and non-licensed nursing personnel, while maintaining and exceeding care and service standards. Provides direction and nursing leadership to care team in accordance to state and federal regulation for skilled and long term care delivery.
Ensures delivery of nursing services to residents according to professionally recognized nursing practices. Monitors nursing care for compliance with the federal, state and local regulations. Participate in the development of resident care plans; modify resident care plans to reflect current health status & ensure proper Medicare documentation as required. Observe for significance change in the resident condition and provide direct care as needed; report to MD & family. Practice and observe all safety rules and procedures adhering to universal precautions and all infection control guidelines; inform DON of any real or potential infection control concerns. Perform other reasonable tasks as assigned by the administrator or DON.
Resident Care & Communication:
Supervise the daily delivery of nursing services within the facility; monitor work of nursing staff to ensure that all prescribed medication & treatments are administered and documented. Monitor for effective communication within & between nursing shifts. Implement and monitor for positive resident relations among nursing staff; respond to resident and family concerns or problems. Maintain & protect resident information at all times. Adhere to HIPPA guidelines. Assists LPN’s and C NA’s with difficult resident assignments.
Develop plan and procedures that will ensure compliance with the regulatory in-service training requirements, Conduct in-service as required. Assist with the recording and monitoring of C.NA in-service hours.
Assist the DON with development of effective staffing and scheduling patterns to maintain continuity of resident care on all shifts within State and Federal regulations; assist with orientation and training of nursing personnel; responsible for evaluation assigned nursing staff, practice positive employee relations. Participates in the hiring, evaluation and performance improvement action planning of staff
Monitor all personnel and conditions for safety and efficiency. Ensure effective communication of the facility polices and procedures and goals relating to safety to facility staff, residents and family members. Initiates investigations and submit reports as require by law and policy. Maintain appropriate record of all resident’s and incidents investigations, findings and action plans relating to safety issues and adverse incidents. Prepare and report to Quality Assurance meeting monthly. Maintain and practice confidentially of resident information at all times.
